Authorization is granted to perform maintenance and/or repair services on your equipment.
PLEASE REVIEW AND ACCEPT THESE TERMS AND CONDITIONS. A TECHNICIAN WILL BE DISPATCHED ONCE THESE TERMS ARE ACKNOWLEDGED AND ACCEPTED
Rapid Road Service, and its contracted service providers deliver emergency roadside assistance for various commercial equipment. We use an extensive network of third-party vendors to diagnose, triage, and repair the specified issues. Please be aware that some vehicles may require further diagnosis at a shop, and you might still be responsible for costs associated with our service and the vendor’s time and mileage.
A minimum down payment of $250 is required to begin service. This payment will be applied to the total cost of the service event. All sales are final. If Rapid Road Service cannot locate or access the equipment, or if you are not present, a dry run fee may apply. If the equipment has moved or is not at the specified site, a GOA (Gone on Arrival) fee may be assessed.
By accepting these terms, you authorize Rapid Road Service to charge your account for the services as outlined in this Work Order. This payment authorization covers the goods and services described. You confirm that you are an authorized user of the payment method provided and agree to all charges related to this service event. If using a credit card, it must be in the name of your company or match the name registered with the credit card company. If Rapid Road Service cannot provide service or locate a vendor, any collected funds will be refunded.

I hereby authorize Rapid Road Service and its employees or contracted service providers to perform the repair work as described in the estimate, covering all labor, parts, materials, and related charges. I agree that Rapid Road Service or the contracted service provider is not liable for loss or damage to the equipment or items left in it due to theft, fire, or other causes beyond their control. I also agree that Rapid Road Service or the contracted service provider is not responsible for delays due to parts availability, weather, traffic, or police intervention.
I grant Rapid Road Service’s contracted service providers permission to operate my equipment for testing and inspection. Should additional labor, parts, or materials be required, I will be contacted for authorization. I acknowledge and approve any supplemental estimates for additional work. If I choose to stop repairs, I will cover the costs of any completed work, non-returnable parts, or restocking fees. I am entitled to retain any non-returnable parts I have paid for.
